1. Become a Member of the Cryonics Institute |
Becoming a Member of CI is simply a matter of completely filling out a form (the Membership Application)
(including date of birth) and paying the necessary fees to become an Option One or Option Two Member.

2. Make a request for the necessary forms |
All CI Members can use personalized forms/agreements to make cryopreservation arrangements. The personalized
information comes from the Membership Application. The request should specify that the forms are the ones
required to become an SA-Affiliated CI Member. Send the request by letter to:

Currently only residents of North America can become SA-Affiliated CI Members.

3. Complete your personalized agreements and arrange for cryopreservation funding |

4. Agreements approved by CI are sent to Suspended Animation for examination and approval |
After the CI contracted officer has examined and approved the agreements, they are sent to SA. The
prospective SA-CI Affiliated Member must provide SA with health status information. If Standby costs
are not being paid for by an insurance policy, the SA-CI Affiliated Member must also arrange for
funding of pre-mortem Standby, either through pre-payment or a special purpose Deployment bank account.
